How do I arrange care quickly after a hospital discharge?
To arrange care after hospital discharge, speak to the hospital's discharge team as early as possible and ask what support will be in place before the person goes home. This is often called discharge planning and may include short-term care at home or a temporary placement. In many cases, the NHS may fund short-term support (often up to 6 weeks) while needs are assessed. After that, longer-term care is usually arranged through your local council and may be means tested. Speak to the hospital discharge team early and confirm what support is in place before leaving.
