How do I move someone into a care home quickly?

Moving someone into a care home quickly requires parallel action. If the person is in hospital, work with the discharge team as they can sometimes facilitate a direct move to a care home. If moving from home, contact care homes directly — many have waiting lists but also have vacancies at short notice. At the same time, request a care needs assessment from the local council, as this is required if council funding is needed. If the person is self-funding, you can move faster by approaching homes privately without waiting for a council assessment.

Calling five homes on the same day is far more effective than waiting to hear back from one before trying the next.
