What does a care plan actually contain?

A care plan sets out what support someone will receive, how it will be delivered and who is responsible for delivering it. It should include the person's assessed needs, the outcomes they want to achieve, the specific services being provided, who provides them and how often, and how and when the plan will be reviewed. The person and their family should be involved in writing it and must be given a copy. If care is provided by an agency, the agency will also have a more detailed operational care plan for their staff. A care plan must be reviewed whenever needs change and at least annually.

If you have not been given a copy of the care plan, ask for one in writing — you are entitled to it and it is the document that governs what support the person receives.
